Former State of Ohio Employees

Separated State of Ohio employees who opted to receive their W-2/W-2c and 1095-C electronically will need to log in to myOhio with their former 1x account (e.g., 10281111) to access their 2022 tax forms. If access is denied, call the Ohio Administrative Knowledge System (OAKS) HelpDesk at 614-644-6860, or toll free at: 1-877-644-6860. Select Option 2, then Option 1.

Have the following items ready when calling:

  • Your eight-digit State of Ohio user ID (e.g. 10281111).
  • A valid email address that you can easily access. This address is used to notify you on the status of your case (if applicable).
  • The approximate termination date of your state employment.

The goal is to activate your account while you are on the telephone and ensure that you can access your 2022 tax forms. If this is not possible, the OAKS Helpdesk will escalate the case and you will receive email notifications as the case status is updated. Due to higher-than-normal call volume, escalated cases could take up to five business days for resolution.
